In checking the box below I agree that is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
You must notify Academy by email ten (10) days in advance to cancel Auto Pay tuition. Payment is due regardless of attendance. There are no pro-rations of tuition for missed classes. Credit will only be applied if the Freeze Notice form is provided before the freeze month.
Students may freeze their account for holidays, sickness, or other personal absences. You must notify Academy by submitting a "Freeze Notice" (email) ten (10) days in advance of the month you are to be away. Your membership will be temporarily frozen and extended by the amount of time you have requested to have your membership paused. A minimum of 2 weeks is required to freeze account.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending MAUI JIU-JITSU ACADEMY and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the academy may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MAUI JIU-JITSU ACADEMY professors, staff, employees, students, and and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at MAUI JIU-JITSU ACADEMY. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the academy, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MAUI JIU-JITSU ACADEMY , its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any academy program.
